Add open source process documentation

Description

To facilitate the maintenance of this project, we should add open source process documentation, and centralize all project documentation to the repo (such as the vision doc).

Subtasks

• Add CONTRIBUTING.md
• Add CODE_OF_CONDUCT.md
• Add vision doc to the repo to replace google doc as single source of truth
